acute pain in left great toe

For several months(6-9)I have been experiencing sharp pain in my left great toe specifically when laying down for the night. It suddenly appears out of no where and/or when the blanket touches it and when I cross my right leg over my left anywhere between my knee and foot. Recently the pain has spread to the toe next to the great toe. The pain is so severe that it awakens me. Your response is appreciated.

Truthfully I am stumped. Usually the type of pain you describe would occur when ambulating and active, not when you go to bed, unless your pain is nerve like in origin (burning, tingling, etc.) This may occur at bedtime due to pressure on the nerves coming out of the lower back which may be irritated by the position in which you are sleeping.
I would think your symptoms are worthy of a trip to the doctor just to make sure it is nothing.
