Hi,
If you have been taking medication for gout for four days and your symptoms have not begun to resolve, there is a very good chance you do not have gout. Your problem, on the surface at least, sounds more like an infection than gout.
You should call your doctor and let he or she know that you are still in pain and perhaps rethink the diagnosis.
