Sensation without stimuli

Most nights while in bed I have the sensation you would expect if a large marble were being pressed into the underside of your of your first and second toes (or second and third). It is along the shafts, between the toe pads and the sole of the foot. It's very noticeable but not pain, and it is not numbness (in that I can feel touch to the area during an episode). Left foot only. First noticed years ago when putting on shoes: I'd take the shoe right off again expecting to find foreign object or at least a crease in my sock, but nothing there. Seems neurological. I have had no real injury (occasional sprained ankle), but do have RLS. Wondering if it might be symptomatic of something else.

You do not mention if this is occurring on both your feet or not. Assuming you only get this sensation at night, immediately two thoughts come to mind.
1. Your symptoms may be the result of a nerve impingement in your lower back where the nerves that innervate the bottom of your foot come out the spinal column. This could be aggravated by your sleeping position.
2. I do not know what you do for a living but during the day are you putting excessive pressure on the ball of your foot such as from climbing ladders or driving a stick shift and using the clutch?
The kind of sensation you describe is usually the result of inflammation in the area which gives the feeling of "fullness" in the area which might be more noticeable at night then during the course of the day.
The only way to know if your "sensation" is coming directly from the foot or from your lower back would be to see a doctor who after testing and taking a medical history should be able to narrow down where your symptoms are coming from.
